You're getting 4 points in total as that's the mark you've got in ordering question.
So that's ordering question's problem.

* Sometimes ticks appear spontaneously, this was the case for questions 21 and 22. Sometimes they appear after the second or third resubmission when I was working on other issues with bugs.

that was fixed under http://bugs.lamsfoundation.org/browse/LDEV-2645.
all of these errors were affected by the ordering question problem.
except for 1, which I wasn't able to replicate. hopefully, it's been fixed too
