I am posting this here in case others are afflicted by this problem.

Symptoms of this problem:
- while attempting to add a lesson (or in any one of a number of other situations) a blank page appears instead of the desired screen
- pages show they are downloading data but nothing happens
- error logs and activity logs show no apparent error states.

Be careful that the URL you are using to access the site exactly matches that in the System Admin server properties.

e.g. if you have set "http://www.mydomain.com:8080/lams", you will still be ABLE to access lams at "http://mydomain.com:8080/lams" but you will see multiple weird errors such as those posted above that will drive you spare.

LAMS feature suggestion: either invalidate wrong URL forms, or correctly support "www." as optional in the URL.
