That's because your user Sara does not have teacher permissions in any of the courses.

In order to give Sara teacher permission in a course, login as sysadmin and go to course management, select the course you want Sara to have teacher rights and then add Sara as a Author and Monitor, save and now login as Sara and you should be able to see the button to add lessons.
