Check your LAMS log? /path/to/jboss-4.0.2/server/default/log/server.log and lams.log, localhost_access.2009-10-02.log... there might be an error at the time you made the request. If those are ok, some general debugging tips are to check Apache's error log, turn on Moodle debugging to full (Admin->Debugging), check that all lamstwo tables have been created (mdl_lamstwo, mdl_lamstwo_lesson, mdl_lamstwo_grade).
